There are helpful articles posted at the website of the International Association of Christian Chaplains. While focussed primarily on the work of chaplains, some of the articles have content that can be helpful to lay leaders and elders.
The following Prayer Guide for Presbyterian Elders is a collection of comments and suggestions from elders who have learned, and are learning, about prayer in their regular visitation. The Committee On Church Growth is indebted to the Session of Grace Church in Orleans, Ontario, for their efforts and co-operation in the preparation of this booklet.
This material was originally published as a booklet by the Committee to Double in the 80's in 1982.

This website of St. Mark's Cathedral in Seattle usually has one or two good articles on Pastoral care written by the Canon Pastor, Herbert Anderson. Anderson is a retired Professor of Pastoral Care and Counselling (University of Chicago School of Divinity).
Originally published in 1951 by one of the founders of the 20th century pastoral care and counselling movement, Russell Dicks, this website provides the full text of what is still a great resource when wanting to learn more about Pastoral Care, especially for lay people and elders.
